In the current healthcare landscape, community hospitals are under pressure to do more than simply deliver care. They are expected to improve affordability, transparency, and outcomes for patients, employers, and their own workforce to stay in business.

Sauk Prairie Healthcare has embraced that challenge, and in doing so, earned recognition with a Healthcare Transformation Award at The Alliance’s 2026 Spring Symposium for their dedication to improving the cost and accessibility of healthcare.

Serving more than 45,000 people in south-central Wisconsin, Sauk Prairie Healthcare has become a standout example of how a community hospital can lead meaningful change. Through bundled payments, transparent pricing, and innovative benefit design, Sauk Prairie is proving that high-quality, affordable healthcare is achievable at the local level.

A Community Hospital with Regional Impact

Sauk Prairie Healthcare is a 36-bed, nonprofit acute-care facility located in Prairie du Sac, Wisconsin. Founded in 1956, the hospital offers a comprehensive range of services, including:

Serving communities from Poynette to Lone Rock and Plain to Black Earth, Wisconsin, Sauk Prairie Healthcare combines broad regional access with the agility and close community relationships that define high-performing rural hospitals.

This balance of scale and responsiveness has allowed Sauk Prairie to remain focused on what matters most: delivering high-value care.

A Leader in High-Value Healthcare

Over the past five years, Sauk Prairie has shown consistent dedication to managing the total cost of care, including:

Rather than accepting traditional reimbursement models as fixed, Sauk Prairie Healthcare has been willing to explore alternative payment arrangements that improve affordability and predictability.

A Long-Standing Partnership with The Alliance

Sauk Prairie Healthcare’s success has also been shaped by its deep and collaborative relationship with The Alliance.

As both a provider and employer-member, Sauk Prairie exemplifies what healthcare transformation looks like when stakeholders align around shared goals.

Sauk Prairie Healthcare joined The Alliance as a provider in 1994 and became an employer-member in 2014, making it one of the cooperative’s most engaged and forward-thinking partners.

Over the past three decades, Sauk Prairie has consistently been an early adopter of innovative programs and payment models.

Early Leadership in Performance Measurement

From 2003 to 2007, Sauk Prairie was among the first providers to partner with The Alliance to track performance metrics, including hospital readmission and cesarean delivery rates.

At a time when quality measurement was still evolving, Sauk Prairie Healthcare demonstrated a willingness to embrace transparency and accountability.

Participation in QualityPath®

From 2016 to 2020, Sauk Prairie Healthcare was also one of the first providers to qualify for and participate in The Alliance’s QualityPath® program. The program focused on bundled payments and patient warranties for post-surgical complications.

Bundled payments provide a single, upfront price for an episode of care, helping eliminate billing surprises and giving employers and patients better cost forecasting.

Sauk Prairie Healthcare had the highest volume of joint replacements within the program, reflecting both the strength of its orthopedic services and its commitment to delivering reliable outcomes.

Importantly, the hospital also offered the QualityPath® program to its own employees, demonstrating Sauk Prairie Healthcare’s willingness to stand behind the same high-value care model it promoted to others.

A Pioneer in Price Transparency and Predictable Reimbursement

Long before healthcare price transparency became a national conversation, Sauk Prairie Healthcare was already working to build a more predictable reimbursement model.

Early Adoption of Medicare-Based Pricing

In 2010, Sauk Prairie became an early adopter of:

Since 2022, Sauk Prairie has been among a small group of providers using full Medicare-based reimbursement methodologies, including outpatient groupers.

For employers, this means:

This willingness to rethink reimbursement methodology is central to Sauk Prairie Healthcare’s transformation story.

Expanding Bundled Payment Arrangements

Beginning November 1, 2025, Sauk Prairie Healthcare expanded its bundled payment offerings, giving patients and self-funded employers more predictable pricing for common procedures and services.

Their bundled arrangements include more than 37 common surgical procedures and tests, including:

The hospital has also expressed interest in developing maternity bundles in 2026, further demonstrating its willingness to expand value-based care into new service areas.

A Recognized Preferred-Value Provider

Sauk Prairie was the third hospital to be recognized as a Preferred-Value Provider through The Alliance.

This designation reflects the hospital’s ability to consistently deliver:

For employers and employees, this means easier access to providers who have been proven to deliver better value.

For self-funded employers in particular, Sauk Prairie’s pricing model reduces one of the biggest challenges in health benefits: unpredictable costs.

Leading by Example Through Employee Health Benefits

Sauk Prairie Healthcare does not just promote high-value healthcare externally; it applies the same principles to its own workforce.

As an employer, Sauk Prairie Healthcare has built an innovative employee health plan that reflects the organization’s values of access, affordability, and quality.

Tiered Benefit Plan Design

Sauk Prairie encourages employees and dependents to use Tier 1 providers, including Sauk Prairie itself, for the greatest value. This tiered benefit design helps guide employees toward cost-effective, high-quality care while still allowing them to seek care where they are most comfortable.

The organization also leverages The Premier Network Ruby by The Alliance to provide additional savings opportunities for employees seeking care through UW Health providers.

On-Site Pharmacy

In February 2025, Sauk Prairie opened an onsite pharmacy and redesigned its pharmacy benefits to encourage employees to use this lower-cost option.

This move increases convenience for employees while helping reduce prescription costs for both employees and the organization.

A Holistic Commitment to Employee Well-Being

Sauk Prairie’s healthcare transformation efforts go beyond cost containment. The organization has also invested deeply in employee well-being through wellness programs including:

Wellness4Life Program

Centered around four pillars, Moving More, Eating Smart, Balancing Life, and Staying Well, this initiative promotes physical activity, stress management, healthy eating, hydration, and preventive care.

Vitality Wellness Platform

This digital wellness tool empowers employees to make healthier choices through personalized goals and rewards.

Calm App Access

All team members and their dependents receive free annual memberships to the Calm app, which provides guided meditations, sleep stories, and stress-reduction exercises to support mental health and resilience.

A Model for Community-Based Healthcare Transformation

Sauk Prairie Healthcare’s Healthcare Transformation Award is not the result of one program or one initiative. It reflects years of consistent leadership in affordability, transparency, and collaboration.

Including:

Sauk Prairie has built a model that works for patients, employers, and the broader community. The organization continues to prove that high-quality, affordable healthcare is achievable, one patient, one employer, and one community at a time.
